Observation

C.G.K and B.K have documentation of Basic STARS but there is nothing reflected in MERIT. Provider was informed to follow up with MERIT and request that it be entered into the system for her. C.G.K completed her STARS hours in April 1999 and B.K. completed her in October 2013. All staff were missing a current BBP training. CPR/First Aid missing for C.H. and D.K. Food Worker card missing for C.H., B.K and D.K. C.G.K is missing Safe Sleep reflected in MERIT. She had certificate in her file reflective of 6/17/2023-6/17/2024 but it is not reflected in MERIT. Told to follow up with with MERIT. C.H, B.K and D.K do not have Safe Sleep reflected in MERIT.
